Position Overview:

New Horizons has an immediate full-time opening for an LPN or RN in our Community Mental Health Center which provides rehabilitation services to adults with serious mental illness located in Jefferson City, MO.

 

  • Hours: Full-Time, Monday-Friday 8:00 am – 4:30 pm (no on-call).
  • Training: Must obtain CPI and CPR (will train). Willing to become a CPR/First Aid/AED instructor (will pay for this instructor’s certification).

Job duties include but not limited to:

  • Performing health screenings/assessments/metabolic screening
  • Coordinating healthcare needs of the consumers we serve with internal and external providers to ensure continuity of care
  • Identifying wellness target areas/goals
  • Educating staff and consumers in health and wellness topics
  • Developing and enhancing wellness initiatives
  • Completing post hospital discharge medication reconciliations
  • Training staff in CPR/FA/AED
  • Using information technologies like Care Manager to oversee the health trends of consumers and to follow up on identified trends impacting the health of the population
  • Implement and abide by company policy

 

Requirements:

LPN or RN licensed in MO.
